How the Referral Process Works:

1. You get a prescription for therapy from your child’s primary care physician. The prescription must contain Diagnosis and Treatment Codes, and say “Evaluate & Treat for Occupational, Speech, or Physical Therapy.”

2. Email us the prescription and fill in the 4 New Patient Intake Forms below. We will contact your insurer to see what they will cover. Once we verify, we will let you know if there is any co-pay or other out of pocket expenses you would need to cover.

3. After all forms are received and insurance has been verified, we will contact you to do a full evaluation, send it to your child’s doctor to review and sign, then send the signed copy to your insurer for approval. Your insurer usually approves 6 months of treatment before we have to re-evaluate.

4. The authorization process can take 2-6 weeks depending on insurance company, primary care response time and therapist availability.
